How to Become a Patient

Schedule an initial 15-minute Meet & Greet consultation. This brief introductory visit allows you to get acquainted with Dr. Ayodele, discuss your overall health goals, and ensure our practice is the right fit for your medical needs.
2. Complete Patient Consent
During your first clinical visit, you may be asked to review and sign our standard patient consent form. This allows us to request a transfer of your records from your previous family physician (if you have one). it also gives you full information about our clinic policies, privacy protocols, and approach to care.
3. Complete Preliminary Blood Work
Following your Meet & Greet, we will issue lab requisitions for routine blood work and baseline diagnostics. Completing these tests prior to your comprehensive examination allows us to get a clear, detailed picture of your current health status.
4. Attend Your Complete Physical
Once your lab results are available, you will return to the clinic for a thorough Complete Physical Examination. During this visit, Dr. Ayodele will review your lab findings, complete a detailed clinical assessment, and establish your personalized ongoing health plan.
